Sharing a black-and-white photo of Caleb, the caption reads, "The doctor confirmed today that Caleb passed away from a heart condition called Hypertrophic Cardiomyopathy that went undetected in his yearly checkups. "The girls' hearts were just recently checked and thankfully look the way they should right now. The sadness of losing someone you love is unimaginable," the caption continues. "Thank you to everyone who has reached out, your kindness and support continues to help our family. We miss our baked potato everyday. #calebratelife #celebratelife"

A source previously told E! News that there was a family history of the heart condition. "It appears it runs pretty deep," the insider said.

Caleb's parents first revealed what happened to their son as soon as they received test results shortly after his passing. In response to the outpouring of support, Caleb's family decided to host a digital memorial so fans could mourn.

"Caleb's death has raised many questions about how and why this could happen to a seemingly healthy boy. Sadly, tests have confirmed today that Caleb passed away from an undetected medical condition," they captioned the announcement of their son's memorial, which took place on Oct. 6.

"We'll have more definitive answers in the coming weeks but ask that you help us celebrate his life instead of focus on his death. Due to an outpouring of support and people's hope to be part of his memorial, we have decided to live-stream the ceremony tomorrow at 8pm EDT...Thank you for all your kindness during this difficult time."

Our thoughts and prayers go out to Caleb's family during this difficult time.
