Dr. Paul Nassif Calls a Patient "Unrealistic," a Human Doll Wants Gigantic Boobs and More: 9 Must-See Botched Moments

Drs. Terry Dubrow and Paul Nassif continue to work their magic.

On Tuesday night's all-new Botched episode, Paul fixed up Elaine, a lady who was miserable after she let a non-plastic surgeon operate on her nose. As you can predict, he seriously messed it up. Meanwhile, Terry helped Kayden, a transgender woman whose doc deceived her and gave her bigger breast implants than she wanted. And then there was Blondie, the human doll who was on a quest to look even more "plastic." Yes, you read the correctly.

1. Blondie reveals she's a "real-life doll": Blondie underwent Botox, cheek implants, chin liposuction and five boob surgeries in an effort to look like a human Barbie doll. "As I got to be a teenager, I started to think to myself, ‘Why can't I be a real-life doll?'" she says. "Being a real-life doll, I don't work, but I do have my doll fans who pay for my lifestyle." Although her breasts are a 30J cup size and weigh 7 ½ pounds total, she still wants to increase the size because she desires a more "extreme" look. "I have seen so many doctors, it's crazy," she admits. "And they all tell me they just won't put such a large implant in somebody so small." Hmm…wonder why?

2. Elaine says her doctor punctured a hole in her nose: After fracturing her nose in a car accident, Elaine was having trouble breathing so she went to an ear, nose and throat specialist to help with the problem. He wasn't a plastic surgeon, but he decided to operate anyway to help "fix" a bump. When he removed the bandage, there was a hole in the middle of her nose! "I could feel air coming in and out," she explains. "All I want is a better nose," she says. "It doesn't have to be perfect. I just don't want the hole there anymore."

3. Transgender patient Kayden says her plastic surgeon doubled her breast implants without her consent: Kayden received quite the shock when she woke up from her gender-confirming breast surgery with larger boobs than she requested. "I went to a doctor in Miami," she says. "I asked him for 350 ccs. I wake up from anesthesia and he had given me 700 ccs in each implant." Terry and Paul are appalled. Kayden continues her botched tale by telling them what the doctor told her when she asked why he did that. "He says, ‘Well, you're a transsexual. I figured you'd want to look like a doll. You should thank me,'" she tells them. "That doctor should lose his license in two seconds," Paul says. "That's horrible!"

The oversize implants caused Kayden to develop symmastia, the convergence of the breast tissue in the sternum. After hearing her story, Terry is eager to help, but he is also cautious about the results. "There are limitations to what you can do in a woman's breast and even greater limitations to what you can do in a man's chest," he says.

4. Blondie visits a hypnotherapist: It's obviously tough being a doll, so Barbie makes an appointment with a hypnotherapist to clear and focus her mind before her appointment with Terry and Paul. During the session, she tells therapist Janet exactly how it feels to live the doll lifestyle. "Being a doll, first and foremost, is being plastic, which is why plastic surgery is a big part of my look," she says. "I love my breasts how they don't feel real." Just wait, there's more.

She continues, "I love filling up my lips and my cheeks with fillers and it just feels plastic. It doesn't feel real." Blondie also admits hypnotherapy helps her to block out all the haters. "It keeps me very positive and I don't let those negative vibes and energy consume me like it did in the past," she says.

5. Elaine declines Paul's recommendation for an "elephant trunk": Paul tells Elaine the best option to reconstruct her nose is taking a piece of tissue from her forehead and attaching it to her nose. She would have the so-called "elephant trunk" on her face for about four to six weeks, then he would go back and remove it. "I can get you a better result using this versus something else," he tells her. But despite Paul's medical advice, she refuses that option. "That is a mistake because in the long run, it's the best way to help Elaine's nose," Paul says. "If I were to wake up like that, I wouldn't even know how I would react," Elaine explains. "I'd probably be scared as hell."

6. Blondie tells Paul and Terry she wants to double her gigantic implants: The moment of truth finally arrives for Blondie when she visits Drs. Dubrow and Nassif and tells them about her dream to boost the size of her boobs. "I just want to look like a doll and I want to even go larger," she says. But Paul isn't buying it and skeptically comments, "Even though I never played with dolls, from the ones I've seen, I know they don't look like that." Terry asks her exactly how big she wants to go and she tells him she wants 3,000 to 3,500 ccs. Whoa! After hearing the revelation, Terry says, "Right now, Blondie is standing nicely upright. If she attempts to go to 3,500 cc implants, she's going to fall over." LOL!

7. Blondie admits she doesn't get mammograms: As Terry is examining Blondie's breasts, he notices some irregularities and asks her if she has ever had skin cancer. That's when she reveals she's had a tumor on chest that came back twice! "Blondie doesn't know it yet, but she is already rejecting her implants," Terry says. "She has a full-blown symmastia and epidermal thinning. Not good." Yikes! He also asks how she gets mammograms with such massive boobs and she responds, "Dolls don't get mammograms." Oh really? "That's a very, very irresponsible, dangerous outlook," Paul says. "She needs a mammogram now."

Terry then gives Blondie some rather disappointing news: If she continues to expand her implants, they could burst! Uh-oh. He cautions her not to go any further to avoid complications. "We do not want to see you again in this office with a problem," Paul tells her. But despite their advice, Blondie is still determined to reach her ultimate goal. "I just want to look like tits on a stick," she says.

8. Paul tell Kayden she's "unrealistic": Kayden comes back into the office after her surgery to follow up with the docs because she has some concerns with the lack of definition in her cleavage area. Terry takes full blame for the situation. "I thought that I set your expectations properly and I obviously didn't," he says. "Kayden has an idealized set of breasts in her mind that are, first of all, not possible on someone who is a genetic male and not possible even on a female who isn't born that way to begin with." Paul then asks if he can jump in and add his two cents. "With respect, this is a little bit unrealistic," he says. "What I've seen so far, this is absolutely the best correction I've ever seen for symmastia." In the end, Kayden is satisfied with her improvement and all three of them hug it out!

9. Elaine flaunts her new nose: Although Paul wasn't pleased she didn't choose the "elephant trunk," Elaine was still ecstatic with the results he achieved. "Having my nose done has changed me a lot," she said. "As I look into the mirror at myself, I see a beautiful Elaine." Aww! "I've been a prisoner with my nose and now I'm totally free."
